# Send Your Doctor a Pre-Visit Health Summary Report

> Futurum compiles your symptoms, wearable data, and health history into a structured medical report your doctor reviews before your appointment.

Before your next appointment, Futurum compiles everything you've tracked — your symptoms, wearable data, health patterns, and any notes you've added — into a structured medical report. Your doctor reviews it before you walk in the door, so you skip the recap and get straight to care.

## How it works

<Steps>
  <Step title="Describe your symptoms">
    Speak or type in your own words — no medical terminology required. Describe what you've been feeling, when it started, and anything that makes it better or worse.
  </Step>

  <Step title="Futurum organizes your input">
    Futurum's AI reads your description and organizes it into a structured medical format, identifying duration, patterns, and severity automatically.
  </Step>

  <Step title="Your health data is automatically included">
    Your wearable data, vitals, medication list, allergy information, and relevant health history are pulled in from your connected sources — no manual entry needed.
  </Step>

  <Step title="Review the report before sending">
    Open the report in the app and confirm everything looks accurate. Add any additional notes you want your doctor to see before you send.
  </Step>

  <Step title="Send it to your doctor with one tap">
    Select your doctor or clinic and send the report. They receive and review it before your appointment, so your visit starts with the full picture already on the table.
  </Step>
</Steps>

## What's included in your report

Every pre-visit report automatically includes the following information, drawn from your tracked data and connected devices:

<CardGroup cols={2}>
  <Card title="Symptoms" icon="notes-medical" href="/patients/health-tracking">
    Your reported symptoms, organized by duration, pattern, and severity so your doctor can immediately identify what's most relevant.
  </Card>

  <Card title="Wearable and device data" icon="watch" href="/patients/wearable-integrations">
    Heart rate readings, sleep patterns, activity levels, and any other data from your connected wearables and medical devices.
  </Card>

  <Card title="Medications and allergies" icon="pills" href="/patients/health-tracking">
    Your current medication list and known allergies, giving your doctor a complete clinical picture before the visit.
  </Card>

  <Card title="Recent health trends" icon="chart-line" href="/patients/health-tracking">
    Patterns and changes in your health data over time — the kind of context that's easy to forget but critical for accurate care.
  </Card>
</CardGroup>

You can also add free-form notes to any report — questions you want to ask, concerns you want to flag, or anything else you'd like your doctor to know.

## Why it matters

Brief appointments don't leave much room to catch your doctor up on everything that's happened since your last visit. Pre-visit reports remove that constraint entirely:

* **No more trying to remember everything in a 15-minute appointment.** Futurum captures the patterns, timing, and context that are easy to forget when you're sitting in the exam room.
* **Your doctor is fully prepared before you arrive.** They've already read your report, reviewed your data, and can walk in with informed questions instead of starting from scratch.
* **More time for treatment and your questions.** When the recap is already done, your appointment can focus on what actually matters — your care.

## From your words to a structured report

Futurum transforms plain-language descriptions into clinical format automatically. Here's an example of what that looks like:

| Your words | Structured report output |
| -------------------------------------------------------------------------------------------------------------- | -------------------------------------------------------------------------------------------------------------------------- |
| *"I've been having headaches for the past week, usually in the afternoon. They feel worse when I skip lunch."* | **Symptom:** Recurring headaches<br />**Duration:** 7 days<br />**Pattern:** Afternoon onset<br />**Trigger:** Meal timing |

Your doctor receives the structured version — clear, organized, and ready to act on.

<Tip>
  Send your report at least 24 hours before your appointment so your doctor has time to review it.
