What Data We Store
Depending on how you interact with Futurum Health AI, we may store the following categories of information:- Protected Health Information (PHI) and billing data, such as patient identifiers, diagnoses, procedures, clinical notes, insurance coverage, claim adjudication details, and remittance advice used to support claims preparation, coding, submission, and payment reconciliation.
- Account and user information, such as organization name, billing contact, account credentials, user name, email, role, and professional credentials.
- Technical and usage data, such as IP address, browser type, operating system, device identifiers, usage logs, timestamps, error reports, and analytics.
- Deidentified and aggregated data derived from the above, which cannot reasonably be used to identify an individual.
- Communications you send to our team, such as emails and support requests.
Your Right to Request Deletion
You may request that Futurum Health delete personal information we hold about you. Once we receive and verify your request, we will delete your data from our active systems within a reasonable timeframe, typically within 30 days.Some information may be retained after a deletion request when required by law, for legitimate business reasons, or to comply with HIPAA recordkeeping obligations. For example, billing and claims records may need to be retained for regulatory, audit, or tax purposes. Where retention is required, we will limit use of that data to those specific purposes. How to Submit a Deletion Request
To request deletion of your data, email us at the address below from the email address associated with your account.Email Your Deletion Request
- Subject line: “Data Deletion Request”
- Your full name and the email address associated with your account
- The organization or clinic you are affiliated with, if applicable
- A brief description of the data you want deleted, or a request to delete all of your data
What Happens Next
- Verification. We will verify your identity to protect against unauthorized requests. We may ask for additional information to confirm your identity or your relationship to the account.
- Review. We will review the request and determine what data can be deleted and what, if any, must be retained for legal or regulatory reasons.
- Confirmation. Once your request is processed, we will send a confirmation email summarizing what was deleted and what (if anything) was retained and why.
